This is a small covered box from the Cherished Teddies collection. The box says "MOM" and has a blue bow. The bear sitting on top is wearing a hat and a gold necklace.

This 3" tall covered box was introduced in 2001. Originally it came with a gold-colored necklace inside, but it no longer includes the necklace or the original packaging.
